The first step towards creating a beautiful smile and achieving optimal dental health is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient please visit our Patient Forms page to download our new patient bundle.  Printing the forms, filling them out, and bringing them with you will allow us to attend to your dental needs in a more timely manner then completing them upon arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, most major credit cards, and care credit.   Payment is expected at the time services are performed unless previous arrangements have been made.  Feel free to contact our office if you want to discuss financial arrangements. 

 
Dental Insurance

We accept and process all dental insurances.  Please contact our office for Insurance details.  We look forward to hearing from you.
